Tryout Period Three (3) Days or Less:
1. Prospective "tryout student-athletes" must complete a Tryout Release & Waiver of Liability Form prior to participating in any tryout activities.
The Tryout Release & Waiver of Liability Form will be reviewed by a member of the University of Maryland Sports Medicine Unit.
2. If, for any reason, the prospective "tryout student-athlete" is not medically approved / certified for participation in tryout activities, he/she will be notified by a member of the University of Maryland Sports Medicine Unit.
Costs associated with tests, consultations, and/or medical procedures needed to gain medical approval / certification for the tryout process will be the responsibility of the prospective "tryout student-athlete" and/or his/her parent(s) / guardian(s).
3. If the "tryout student-athlete" is selected to join a University of Maryland athletic team, the following materials must be on file in the University of Maryland Sports Medicine Unit.
Health History Questionnaire Form;
Supplement Notification Form
;
Health Insurance Information & Authorization Form;
Photocopy (front & back) of their Health Insurance Card and their Prescription Benefits Card;
Medical Examination & Authorization Waiver;
3.1 The student-athlete will also be required to undergo a physical examination review conducted by a University of Maryland Team Physician. The pre-participation physical examination will include, but is not limited to, the following:
Medical history review;
Height, weight, and vision screen;
Pulse and blood pressure;
Medical Physical examination; and
Orthopedic screening examination.
3.2 If, for any reason, the student-athlete is not medically approved / certified for intercollegiate athletics participation, he/she will be notified by the University of Maryland Team Physician and/or a member of the University of Maryland Sports Medicine Unit at the end of the pre-participation physical examination.
Costs associated with any tests, consultations, and/or medical procedures needed to gain medical approval / certification for participation will be sent to the student-athlete's primary health insurance for payment first.
Costs not paid by the student-athlete's primary health insurance will be paid by the University of Maryland Sports Medicine Unit.
4. The University of Maryland Sports Medicine Unit reserves the right to refuse medical approval / certification for participation in University of Maryland Intercollegiate Athletics based on the medical opinion of the University of Maryland Team Physician and/or his/her designee. Under no circumstances may a student-athlete seek medical clearance / certification from a private (non-University of Maryland) physician and/or authorized consultant.
Tryout Period Greater Than Three (3) Days:
1. Prospective "tryout student-athletes" must have the following materials completed and on file in the University of Maryland Sports Medicine Unit in order to be considered for medical clearance / certification:
Health History Questionnaire Form;
Supplement Notification Form

Health Insurance Information & Authorization Form;
Photocopy (front & back) of their Health Insurance Card and their Prescription Benefits Card; and
Medical Examination & Authorization Waiver;
